How We Make Care Personal To You
We understand that every person's needs are different, and every health issue is different. When conducting our client’s personalized care plan with our client and/or responsible party to the client, we customize specifically to you or your loved ones' needs.
We are aware that client needs do change, whether it be over the course of months or even days. Our clients are regularly scheduled for an RN visit for reassessment within 90 days of their initial assessment, but if the clients needs do change within that timeframe, an assessment can be scheduled and performed on an as needed basis. This can be requested by the client or suggested by in-home caregivers; as they have first hand insight of the client’s conditions of change.
We also understand that our clients lives change daily and their needs as well. Our staff is designed with client’s flexibility in mind. We are able to increase or decrease the amount of time our in-home caregivers spend with your love ones according to their needs. We also acknowledge the benefit of having the same caregiver and building rapport with the client and strong stable relationships. Rapport building amongst caregivers and families support your loved ones return to a close to normal in-home situation. This affords your family the opportunity to get back to being a family.
